Please find below guidelines on how to proceed with ticket re-issues if your customers have lost (forget/stolen/lost/mislaid/lost in the post) their tickets.
If the ticket had been lost, please fill out the 'Form of Indemnity' (F.O.I.) together with the customer and re-issue the ticket. A copy of this form can be obtained and downloaded online if you click here.
In order to have a new ticket issued against a British Airways 'Form of Indemnity' (F.O.I.) you must provide us the following information in the form: - The original ticket number
- Date of issue
- Form of payment
- Full itinerary details
- Original fare and fare basis
- Complete taxes, fees and charges breakdown
- Any deal number/tour code if applicable
- The IATA number of the issuing office/your office
A new ticket can then be issued, with a charge levied to the travel agency of GBP 25.00 (converted into respective local currency) to cover the F.O.I. fee. It would be advisable to endorse the new ticket to show that it has been issued in exchange for the old lost ticket - to make it apparent should any airline officials need to examine the ticket. Travel Agencies can do this ticket re-issuing without reference to British Airways providing they can complete all the necessary paperwork directly with the passenger who has lost the ticket, and that all paperwork is submitted at the earliest opportunity.
Note: An F.O.I. can only be used against restricted fare tickets - all full fare tickets must be submitted for refund and a completely new ticket must be issued. A F.O.I. only applies from the country where the passenger is departing.
Important - please note:
Please bear in mind that you can always avoid troubles and extra workload with lost, forgotten, mislaid or stolen tickets if you issue straight away an e-ticket for you customers!
